Introduction
The Nebraska Department of Motor Vehicles publishes a one page sheet headed Power of Attorney (For Vehicle/Motorboat Only), and it is what an owner signs to put someone else in their place when the trip to the county treasurer office is not possible. It is spare. You name the person you appoint, state that it is for the purpose of transferring ownership or making application for title and registration, describe the vehicle or motorboat by year, make and VIN or HIN, and then two owner blocks follow, side by side, each with a printed name, a signature and a date. That is the whole form. It carries no notary acknowledgment block, and it carries no penalty of perjury certification either, so Nebraska asks for nothing more than the owner printed name, signature and date. The line that gives the authority reads: with full authority to sign on my behalf all papers and documents and to do all that is necessary to this appointment. One footnote matters more than its size suggests. An asterisk on the title of the form carries the warning that dealers may only use this form when making application for a duplicate title or when odometer is exempt from reporting, which is how Nebraska keeps the sheet clear of the federal odometer rules. The DMV forms index lists it as Power of Attorney - Limited, and the posted revision line reads Revised 9/2024. The DMV does not print a form number on it. Titles in the State of Nebraska are issued at the local county treasurer offices, so the completed sheet goes there with the title or registration application rather than to Lincoln. Key Things to Know
- 1
The Nebraska document is the Department of Motor Vehicles sheet headed Power of Attorney (For Vehicle/Motorboat Only), and it appoints an attorney in fact for the purpose of transferring ownership or making application for title and registration for the one vehicle or motorboat described on it.
- 2
No notary is involved. The form prints no notary acknowledgment block, and unlike some states it prints no penalty of perjury certification either. Each owner gives a printed name, a signature and a date, and that is all the form asks for.
- 3
The DMV prints no form number on this sheet. The forms index on the DMV website lists it as Power of Attorney - Limited, and the posted revision line reads Revised 9/2024, so ask for it by title and revision rather than by number.
- 4
The asterisk on the title carries a real restriction: dealers may only use this form when making application for a duplicate title or when odometer is exempt from reporting.
- 5
Motorboats are covered by the same sheet. The vehicle block asks for year, make and VIN or HIN, with no model, body type or title number field, so copy those three entries from the certificate of title.
- 6
Two owner blocks are printed side by side, headed Vehicle/Motorboat Owner 1 and Vehicle/Motorboat Owner 2, so a co-owner shown on the Nebraska title signs the same sheet.
- 7
Titles in the State of Nebraska are issued at the local county treasurer offices, so the signed form goes there with the title or registration application. Apportioned vehicles are titled through the Motor Carrier Services Division instead.

Nebraska Requirements for Vehicle Power of Attorney
DMV Power of Attorney (For Vehicle/Motorboat Only)
Nebraska uses the Department of Motor Vehicles sheet headed Power of Attorney (For Vehicle/Motorboat Only), with the posted revision line Revised 9/2024. The DMV forms index lists it as Power of Attorney - Limited. The DMV prints no form number on it, so match it by title and revision date rather than by number.
No Notary Block, and No Perjury Certification Either
The Nebraska form prints no notary acknowledgment block anywhere, and unlike some states it prints no penalty of perjury certification in its place. Each owner block asks for Name (Please Print), Signature and Date, and the sheet ends there, so the form can be completed without a notary appointment.
Purpose: Transfer of Ownership or Title and Registration
The appointment on the form is for the purpose of transferring ownership or making application for title and registration to the described vehicle or motorboat, and the authority line reads: with full authority to sign on my behalf all papers and documents and to do all that is necessary to this appointment.
Three Vehicle Fields: Year, Make, VIN or HIN
The Nebraska sheet identifies the vehicle by Year, Make and VIN/HIN only. There is no model, body type, plate or title number field. The identification number is therefore the only entry tying the authority to one vehicle, so copy it character by character from the certificate of title.
Two Owner Blocks on One Sheet
The form prints blocks side by side headed Vehicle/Motorboat Owner 1 and Vehicle/Motorboat Owner 2, each with a printed name, a signature and a date, so a co-owner shown on the Nebraska title signs the same page. Use the name as it reads on the title.
Dealer Use Limited by the Printed Footnote
The asterisk after the title of the form carries a footnote that reads: dealers may only use this form when making application for a duplicate title or when odometer is exempt from reporting. If the transaction is neither of those, a dealership needs a different document, so ask before you sign.
Motorboats Covered by the Same Form
The subtitle of the form covers a vehicle or a motorboat, and the identification field accepts a VIN or an HIN, so a Nebraska owner uses the same sheet for a boat as for a car. The appointment language does not change between the two.
File It at the County Treasurer Office
The Nebraska DMV states that titles in the State of Nebraska are issued at the local County Treasurer Offices, so the signed form goes there with the title or registration application the attorney in fact signs, rather than being filed on its own. An apportioned vehicle is titled through the Motor Carrier Services Division instead.
